PHYSICAL EXAMINATIONS OF STUDENTS

In accordance with RSA 200:32, physical examinations, including vision and hearing tests, will be required for all children in Grade 1 (or upon later entry into school, unless evidence of examination is provided by the sending school district).

In accordance with the standards of the New Hampshire Interscholastic Athletic Association, any student in Grades 9 through 12 who participates in athletics, including cheerleading, must have an examination for his/her sport and written consent from the parent prior to the start of any practice.  It will be the parent's financial responsibility to have the examination completed.  An examination for sports will be required when the student starts his/her athletic career in the school and will be good for four years unless an injury or illness requires it to be repeated.  Each year thereafter, the parent/guardian must complete a health questionnaire and sign a consent form before the student may start practice.
