REGULATION ON STUDENT CUMULATIVE RECORDS
Do not destroy records of handicapped children who are on SPEDIS. Records must be permanently kept intact for any child who is on SPEDIS, who is being considered as a handicapped student, or who has been discharged as a handicapped pupil.
A Pupil's Record Folder Should Contain:
Copy of Report Cards
Covers of Tests, Primary Reading Profile, Readiness, Economy, Addison Wesley, Gates McGinite (properly filled out)
Completed Skills Checklists
Pupil Reading Record Sheets
Anecdotal Records - signed and dated (non-judgmental)
When Applicable:
Referrals
Referral Reports
Placement Papers
SPEDIS Forms and SPEDIS coding on Folder
Principals and Assistant Principals are responsible for these records.
The following should be sent to Grade 6, (Middle School) with the Student Folder:
1. Pink Pupil Reading Record Sheets
2. Green Pupil Reading Checklist
3. I.T.B.S. Pupil Profile Sheet
4. The School Copy of Green Permanent Record Sheet
5. Progress Reports
Green Permanent Record Sheet information will be transferred to the Duff Permanent Sheet after Grade 7.
Grade 8 - Everything is transferred to the permanent Record Card.
The Cumulative Record Folder is to be used through Grade 12, and kept for one year, at which time, it is discarded. Handicapped folders should be kept indefinitely.
Nurses keep separate Health Records.
