Student Registration
To register a student for school, grades K-12, parents/ guardians need to complete a Student Registration Packet and submit it along with a birth certificate, proof of residency, immunizations records and proof of physical. This may be completed online and then e-mailed. You may also print out the registration packet and or dropped off in-person or mail to the appropriate address provided below.

Along with the completed registration packet, you must also email, mail, or deliver in-person copies of the following:
A copy of the child’s birth certificate
Immunization records
Proof of residency
Documentation of recent physical exam
Legal/ Court paperwork, as applicable
You may scan the documents, or take a photo of the documents and include them with your registration packet.
The law mandates that no child may start school until birth certificate, immunization records and physical exams have been processed by the school department. Your child will not be assigned to a school until all registration paperwork is complete (birth cert., immunization records, completed registration packet).
